The MDS Coordinator will be responsible for the coordination and/or completion of all MDS (Minimum Data Sets) assessments.
Job Requirements / Qualifications
- Duties might include; the MDS Coordinator will capture significant changes in resident’s status; conducts quarterly review and completion of MDS. The MDS Coordinator will be responsible for communicating with all departments regarding changes in dates and the need for assessments. The MDS Coordinator will maximize case mix data to improve reimbursement; maintains medical records (MDS) and documentation to support the MDS. Participate in regular UR Meetings and other meetings as assigned by the DON and/or Administrator. Completes significant changes in resident’s status; coordinates completion of required nursing assessments upon admission and quarterly; conducts quarterly review of each plan of care; Reviews daily orders for acute/advanced care planning. Assist with staff education on ADL scoring and documentation.High school diploma or equivalent.
- Previous experience working with older adults preferred.
- Previous experience with MDS is preferred.
- Current and in good standing licensure as Registered Nurse in state of Ohio.
Part-time position on day shift at our Skilled Nursing Home.
